Therapeutic Plasma Exchange (TPE) is a major medical procedure. It processes your entire blood volume—often 1.5 times over.
Just as a marathon runner wouldn't run a race without carb-loading and hydrating, a TPE patient shouldn't sit in the chair without nutritional preparation.
What you eat (and drink) in the 24 hours before and after your session determines how you feel.
- Well-prepared patients: Feel energized, have easy vein access, and walk out smiling.
- Poorly-prepared patients: Feel dizzy, faint, and struggle with "beeping" machines due to dehydration.

Phase 1: Pre-TPE (24-48 Hours Before)
Goal 1: Hydrate, Hydrate, Hydrate
This is the #1 rule.
- Why: TPE relies on blood flow. If you are dehydrated, your veins collapse (making access painful), and your blood is thicker (causing machine pressure alarms).
- The Plan: Drink 3-4 liters of water the day before.
- Avoid: Alcohol and excessive caffeine (which dehydrate you).
Goal 2: Calcium Loading
During TPE, we use an anticoagulant called Citrate. Citrate binds to calcium in your blood to prevent clotting in the machine. This causes a temporary drop in your calcium levels, leading to tingling lips or fingers.
- The Plan: Eat calcium-rich foods.
- Dairy (Yogurt, Cheese, Milk)
- Dark Leafy Greens (Spinach, Kale)
- Almonds
- Tums (Calcium Carbonate): Taking a Tums the morning of your procedure is a great hack.
Goal 3: Iron-Rich Meal
TPE can cause a very small loss of red blood cells (residual in the tubing). To keep your hemoglobin strong:
- The Plan: Eat Red Meat, Lentils, or Spinach.
Phase 2: The Morning Of
Do NOT fast. Unless your doctor specifically told you to fast for a blood test before the procedure, you should eat a solid meal. TPE burns calories and can lower blood sugar.
The Perfect Pre-TPE Breakfast:
- Protein + Complex Carb: Scrambled eggs with toast, or Oatmeal with protein powder.
- Fluid: 2 glasses of water.
- Skip: Greasy, heavy foods (bacon, fried potatoes) that might make you nauseous if your blood pressure fluctuates.
Phase 3: Post-TPE (Recovery)
You just lost a lot of plasma proteins (though we replaced the Albumin/volume). Your body needs building blocks to regenerate its own immunoglobulins and clotting factors.
Goal 1: Protein Refuel
Your liver needs amino acids to build new proteins.
- The Plan: High-quality protein. Chicken, Fish, Steak, Tofu, Protein Shakes.
- Timing: Eat a protein-rich meal within 2 hours of finishing.
Goal 2: Rest and Electrolytes
You might feel tired. This is normal "post-exertional" fatigue.
- The Plan: Drink an electrolyte drink (LMNT, Gatorade, Coconut Water).
- Avoid: Alcohol for 24 hours (your liver is busy making proteins; don't stress it with alcohol).
Sample Menu
Day Before:
- Breakfast: Greek Yogurt with berries (Calcium + Protein).
- Lunch: Spinach Salad with Grilled Chicken (Iron + Protein).
- Dinner: Salmon with Asparagus (Healthy Fats).
- Drinks: Water bottle in hand all day.
Morning Of:
- Breakfast: 2 Eggs, Toast, Glass of Milk.
- Supplements: Calcium supplement (Tums).
Post-Procedure:
- Snack (in clinic): Protein bar + Juice.
- Dinner: Steak or Lentil Stew (Iron + Protein) + Electrolyte drink.
