Preparing for Your First TPE Session: What to Expect
Your first Therapeutic Plasma Exchange (TPE) session is a big step toward better health. Whether you are treating a neurological condition or pursuing a longevity protocol, preparation is key to a smooth and comfortable experience.
At VIP TPE, we want you to walk in feeling confident and relaxed. Here is your ultimate guide to preparing for your first appointment.
The Day Before: Hydrate, Hydrate, Hydrate
The success of TPE depends heavily on your veins.
- Why? Dehydrated veins are small and flat, making them hard to access. Hydrated veins are plump and bouncy.
- The Goal: Drink at least 8-10 glasses of water the day before your procedure. Avoid excessive alcohol or caffeine, as these dehydrate you.
The Morning Of: Fuel Up
Do NOT Fast. This is not surgery.
- Eat a Good Meal: Have a solid breakfast or lunch. Include calcium-rich foods (yogurt, cheese, milk, leafy greens) because the procedure can slightly lower your calcium levels.
- Dress for Comfort: Wear loose-fitting clothing with sleeves that can be easily rolled up above the elbow. The clinic can be cool, so bring layers (we provide warm blankets!).
- Bathroom Break: Use the restroom right before we start. You will be hooked up to the machine for 2-3 hours. (We have bedpans/urinals for emergencies, but avoiding them is better!).
What to Bring
Since you will be seated for a few hours, bring entertainment:
- Headphones and a smartphone/tablet (movies, podcasts, audiobooks).
- A book or e-reader.
- A snack (though we provide snacks too).
- A sweater or socks if you get cold easily.
Step-by-Step: The Procedure
1. Check-In and Vitals You will meet your nurse, who will check your blood pressure, pulse, and temperature. We will review your labs and answer any last-minute questions.
2. Access (The Needle Stick) You will sit in a comfortable reclining chair.
- Two IVs: Typically, we place one needle in each arm. One draws blood out, the other returns it.
- Squeeze Ball: You may be asked to squeeze a stress ball with the "draw" hand to keep blood flowing briskly.
3. The Run The machine starts. You will see your blood moving through the tubing into the centrifuge.
- Separation: The machine spins the blood. You might see a bag filling with a yellow fluid—that's your old plasma (the "bad stuff").
- Replacement: A clear bottle of Albumin (the "good stuff") hangs alongside. This replaces the plasma we remove.
- Sensation: You won't feel the spinning. You might feel a coolness in the return arm (since fluids are room temp). If you feel tingling in your lips (calcium drop), tell the nurse immediately—we have Tums ready!
4. Completion Once the target volume is exchanged (usually 2-3 liters), the machine stops. We rinse the tubing with saline to give you back every drop of your red blood cells.
- Disconnect: Needles are removed, and pressure bandages are applied. Keep these on for at least 4 hours.
Post-Procedure Care
The Rest of the Day:
- Take it Easy: Go home and relax. No heavy lifting or gym workouts.
- Hydrate: Continue drinking water.
- Monitor: Watch the needle sites for bleeding (rare). If you feel dizzy, lie down with your feet up.
The Next Day: Most patients feel back to normal. Some report a "TPE hangover" (fatigue) the first time, but this usually improves with subsequent sessions. Others report feeling lighter and clearer-headed.
